Smith College is required by federal law (the Immigration Reform and Control Act of 1986) to verify the identity and employment eligibility of all employees. In order to begin the onboarding process and establish records for Payroll, Human Resources, and to create a Smith Network account, all new faculty members must complete the following forms.

Massachusetts State Tax Withholding Form (M-4)

Each employee must have on file a Federal Tax Withholding Form and a State Tax Withholding Exemption Certificate. For individuals employed and working within Massachusetts you will need to complete the Masachusetts M-4.  For individuals working outside of Massachusetts you will be required to complete a State Tax withholding form for the state in which you reside. Please follow this link to the listing of state tax withholding forms. Please note - some states do not collect state taxes and you may not be required to complete any State Tax withholding form. Those states are also detailed on the linked listing.
