Application Deadline
The application deadline for the Palliative and End-of-Life Care Certificate Program is August 1.
Prerequisites
Applicants should have a master’s degree in social work and at least two years of post-master’s experience. Applicants must be in current practice in palliative or end-of-life care.
Submitting Additional Materials
All materials may be submitted using the application form below. If you need to submit any materials after you have submitted your application, please email them to the Office of Professional Education.
Your application packet must include the following:
- Application form (available online only)
- $40 nonrefundable application fee (included in the application form above)
- Current résumé
- Personal statement (two to three pages) responding to the following:
- Your professional and personal reasons for working in end of life care
- The influences on this choice of practice
- What you hope to gain from this program
- How you will meet the clinical requirements of the program
- How you will incorporate the above in your current workload
- Your willingness to use a consultative clinical supervisor appointed by the program
- The issues you would like to work on
- Two letters of reference (from professional colleagues familiar with applicant's clinical work)
